- Must have A1C Blood Lab Results of (5.7 – 6.4) pre-diabetes
- Must connect Phone App to Report Physical Activity; 5 Absences MAX are allowed
- Bluetooth Digital Scale is REQUIRED for Usage during Program, every week for monitoring

BLUE CROSS BLUE SHIELD = Out of Network Provider
- Depends on your Out-of-Pocket and Deductible being met for Out of Network Providers
- If you’ve met your Out-of-Pocket and Deductible, you will be covered for our Telehealth services, but may have a co-pay.
UNITED HEALTH CARE = Out of Network Provider
- Depends on your Deductible being met
- If you’ve met your Out-of-Pocket and Deductible, you will be covered, but may have a co-pay.
KAISER PERMANENTE = Out of Network Provider
- Depends on your Deductible being met
- If you’ve met your Out-of-Pocket and Deductible, you will be covered, but may have a co-pay.
AETNA = Out of Network Provider
- Depends on your Deductible being met
- If you’ve met your Out-of-Pocket and Deductible, you will be covered, but may have a co-pay.

We work hard with your doctor, our Fitness & Nutrition Professionals, and becoming New Providers of Public/Private Health Insurance companies.
Our Diabetes Prevention Program is fully covered for FREE by Medicare/Medicaid for Missouri Residents ONLY, but in May 2024, it will be covered in all 50 States for Medicare or Medicaid, since we’re a New Health Organization, and have to report our Weight Loss Numbers to CDC, up until then.
With Health Insurances such as, Blue Cross Blue Shield, United Health Care, Aetna, and Kaiser, we are currently an Out of Network Provider.
If you have not met your deductibles for your Health Insurance, or if it doesn’t cover this program, We ask you to Sign up under our WEIGHT LOSS PROGRAM for $35/monthly, + $50 Deposit for Bluetooth Scale (if needed) as this helps for tracking your Activity, Weight, Food Entries, through our monitoring app, and provide you with excellent service!
Annually each summer in July, we have a Wellness Spa Retreat for the end of your Cohort’s Program if you’d like to attend to celebrate, where you’ll receive a FREE Facial, Massage, Makeover, Photoshoot, and Exercise Classes to enjoy a relaxing weekend, in honor of your Weight Loss Journey, thanks to your new founded Healthy Lifestyle!
