From Participant Success to Research Recognition: Why Support Should Continue After the National DPP

TAT Health Solutions is celebrating two important accomplishments that reflect different parts of our mission: helping participants make measurable improvements in their health, strengthening the support available after the National Diabetes Prevention Program ends, and connecting more eligible adults with diabetes-prevention services.

Cynthia, a recent Cohort 12 graduate, completed our yearlong National Diabetes Prevention Program with meaningful improvements in her weight, blood glucose, and blood pressure. At the same time, Temitope “Temi” Alao-Sanni, RD, MPH, MBA, received recognition for research examining continued maintenance support following the National DPP.

Together, these milestones raise an important question: What happens after someone completes the National Diabetes Prevention Program?

Celebrating Cynthia’s Health Progress

Cynthia recently completed Cohort 12 of the TAT Health Solutions National Diabetes Prevention Program. During her time in the program, she achieved several significant outcomes:

  • A 51-pound weight loss
  • A reduction in A1C from 5.7% to 5.3%
  • Stable blood pressure
  • Completion of the yearlong lifestyle-change program

These results are about much more than a change in appearance or a number on the scale. A1C reflects a person’s average blood glucose level over approximately two to three months. Cynthia’s reduction from the prediabetes range to 5.3% represents meaningful progress in an important health indicator.

Her results also reflect the work that occurs between program sessions: making informed food choices, increasing physical activity, monitoring progress, managing stress, responding to setbacks, and continuing even when every week is not perfect.

The National DPP is designed to help adults at increased risk for type 2 diabetes develop sustainable lifestyle habits. According to the Centers for Disease Control and Prevention, the yearlong lifestyle-change program teaches participants to eat more healthfully, add physical activity to their lives, and improve coping skills.

Cynthia’s experience shows what consistent participation, education, coaching, accountability, and group support can help a participant accomplish.

The Question That Comes After Graduation

Completing a yearlong program is a major accomplishment, but graduation does not mean that healthy living is finished. Nutrition, physical activity, stress management, sleep, and weight management require ongoing attention throughout life.

Participants may also face new challenges after the structured program ends. Schedules change. Family responsibilities increase. Illness, travel, grief, holidays, financial pressures, injuries, and work demands can interrupt established routines. A temporary interruption can gradually become a longer period of inactivity or less consistent eating habits.

Research has found that some weight regain following behavioral weight-loss programs is common, although regain is not inevitable and participants may retain meaningful benefits. In one 24-month follow-up of a lifestyle program, participants regained some of the weight they had lost during the year after the intervention ended, but they remained lighter than they were at baseline. Long-term follow-up of the original Diabetes Prevention Program also demonstrated that the lifestyle intervention continued to provide benefits, even as the difference in average body weight between study groups became smaller over time. Read the Diabetes Prevention Program Outcomes Study.

This is why maintenance should not be treated as an afterthought. Losing weight or improving a health measurement is one phase of the journey. Learning how to protect those improvements during real-life changes is another.

Why TAT Health Solutions Created a Maintenance Program

TAT Health Solutions created the FIT Fam Maintenance Program to provide continued structure after participants complete the National DPP.

The purpose is not to repeat the entire first year. Maintenance helps participants continue applying what they learned while receiving ongoing education, accountability, encouragement, and peer support.

  • Continue monitoring weight, physical activity, and other health goals
  • Recognize small changes before they become larger setbacks
  • Refresh nutrition and physical activity strategies
  • Set new goals after reaching earlier milestones
  • Discuss barriers and problem-solve with a coach and peers
  • Maintain connections with people who understand the long-term process
  • Return to healthy routines after travel, illness, holidays, or stressful periods
  • Celebrate progress that extends beyond the scale

Maintenance does not require perfection. It provides a consistent place to evaluate what is working, identify what has changed, and decide what support is needed next.

The FIT Fam Maintenance Program is available to graduates from all backgrounds. Its purpose is to support participants who want continued help sustaining the habits they developed during their initial program year.

Celebrating Temi’s Research Recognition

This commitment to continued support is also receiving research attention.

Temitope “Temi” Alao-Sanni, RD, MPH, MBA, began working with TAT Health Solutions as an intern and has contributed as a registered dietitian, health educator, and member of our program team. Many FIT Fam participants have learned from her presentations and benefited from her nutrition knowledge.

Temi submitted an abstract examining follow-up maintenance support after the National DPP to the 2026 INDI Fresenius Kabi Research Symposium. Her abstract was accepted for online publication and an in-person poster presentation. Following a competitive review involving more than 160 submissions, it was also selected as one of the symposium’s top-scoring abstracts.

Her recognition is important for several reasons. It highlights the need to look beyond initial program completion, brings attention to the experiences of participants after graduation, and supports further evaluation of approaches intended to promote long-term engagement.

Temi’s development from a TAT Health Solutions intern to a research presenter also demonstrates how community-based health programs can contribute to workforce development, applied research, and improved program design.

Two Accomplishments, One Continuing Mission

Cynthia’s health improvements demonstrate what can happen during the National DPP. Temi’s research recognition draws attention to what may be needed afterward.

These accomplishments are connected by one principle: health improvement is not a short-term project. Participants need practical education during the initial lifestyle-change program, but many also benefit from continued relationships, accountability, and opportunities to strengthen their habits over time.
